69-year-old man with recent diagnosis of a bladder carcinoma. 18F–FDG-PET was performed for staging and showed a well-delineated nodule in the right upper lobe. Axial CT-imaging in lung window setting show a heterogeneous aspect of the nodule with small hypodense foci or bubble-like-lucencies. Although the lesion did not show intense uptake on 18F-FDG-PET, lobectomy was performed based on the suspicious morphology. Histopathologic examination showed a 13 mm invasive adenocarcinoma with predominant lepidic growth
