8F-FDG-PET-CT was performed in a 58-year-old man with unexplained fatigue and weight loss. PET (not shown) showed a lesion in the left apex with moderate uptake, corresponding on CT with a 1.2 cm spiculated nodule. On the axial CT-image in lung window setting a some fine linear strands extending to the pleura (pleural tags) are noted. In mediastinal window setting b a small triangular focus of retracted fat can be seen (white arrow). The nodule exhibits some fine linear strands or pleural tags. Histopathologic examination after lobectomy showed an adenocarcinoma with lepidic and acinar growth with focal invasion to the visceral pleural surface (PL2)
