False positive high DWI signal for cholesteatoma. Non-cholesteatomatous tissue (surgically confirmed non-specific inflammation) at the post-operative mastoid remnant, which demonstrated a high signal (white arrow) on the b1000 image suggestive of residual cholesteatoma. However, b the corresponding ADC map demonstrated borderline high ADC signal (white arrow) and value not supporting the diagnosis of residual cholesteatoma. This case could also have been resolved with a follow-up DWI scan
