Axial CT (a) image through the mid-chest demonstrates direct communication of the right main stem bronchus with the stomach (S) (black arrow) resulting in a broncho-gastric fistula. Several slices inferiorly (b), posterior gastric wall is dehiscent (circle) and communicates with the pleural space (P), resulting in a gastropleural fistula. Note the chest tube within the posterior right pleural space
