Stapfer type I DP with moderate retroperitoneal air (+) plus pneumoperitoneum extending to the scrotum (*) shown by emergency unenhanced CT in a 62-year-old male undergoing ERCP treatment of choledocholithiasis. Laparotomic surgery included duodenotomy and suture of a posterior duodenal wall discontinuity, toilette and drainage of the peritoneal cavity. The subsequent course was uneventful and repeated CT with oral CM at discharge (not shown) excluded extraluminal contrast leak
