A 77-year-old male with known colon diverticular disease. Water-soluble contrast enema (a) initially performed during an acute bout of diverticulitis shows a severe “saw-tooth” hypersegmentation appearance and distal narrowing of the sigmoid colon consistent with the clinical diagnosis, but without information concerning the pericolonic structures. WE-MDCT depicts mural thickening of the sigmoid and distal descending colon (be) with a large abscess (*) with mixed faecal and gas content. Some non-complicated diverticula are seen in the upstream large bowel (arrowheads in d, f). Note the surgical clip from previous resection of a polyp (thin arrow) and endoluminal faecal residues from incomplete bowel preparation. Surgical resection of the involved colon and abscess debridement were performed
