Metastatic ILC of the pancreas in a 53-year-old female presenting with painless jaundice. She was on adjuvant letrozole for Stage 2 ILC of the left breast diagnosed 2 years ago. Coronal contrast-enhanced CT image shows a dilated common bile duct (white arrow) and dilated pancreatic duct (arrowhead)—the double duct sign. The intrahepatic ducts are also mildly dilated. No discrete mass is seen in the pancreatic head on CT or on EUS. Metastatic involvement of the pancreatic head was confirmed on intra-operative biopsy
