This patient with a large T3 MRF+ rectal cancer (yellow arrow in a) presents with a hypointense, highly irregular, intermediate-to-low signal intensity nodule in the mesorectum (red arrows in a and b) that does not interrupt the course of a vein but is irregular enough for us to admit the possibility of an extranodal deposit (mrENTD). The patient also presented with a round, hypointense slightly heterogeneous and slightly irregularly contoured lesion which we believe to be a lymphadenopathy (blue arrow in b)
