An 83-year-old male who presented with acute coma. Noncontrast CT obtained 2 days after admission shows hypodensity in the medial aspects of both temporal lobes, compatible with posterior circulation ischemia. Occipital lobes, brainstem and cerebellum are also involved, and there is relative hyperdensity of the basilar artery . Note preservation of the entorhinal cortex (asterisks), supplied by MCA and anterior choroidal artery branches
