Sagittal and axial T2 WIs (a and b) show a large epidural and posterior paraspinal fluid collection in a patient with L4–5 decompression and fusion. Postcontrast image (c) shows no significant enhancement around the collection. The patient had orthostatic headaches and MRI brain confirmed dural thickening and effusion on FLAIR (d) with diffuse pachymeningeal enhancement (e) consistent with intracranial hypotension due to CSF leak and psedomeningocele formation
